Quality vendors are a key ingredient for a successful event. Below are the guidelines for all Food Court, Beer Garden and activities vendors.

Our sincere thanks to those of you who have generously supported us in the past and we extend a warm welcome to you and our new vendors. Click the link to review the Vendor Packet.

Deadline for on-line registration is September 10, 2012.

I Agree	Vendor Expectations
	Follow all Federal, Arizona and Maricopa County Health regulations. Provide proof of insurance. Keep your booth and surrounding area clean of all clutter and refuse. Require workers to maintain proper sanitary conditions including food handlers wearing sanitary gloves. Limited tables and seating will be provided for the public. Vendor tents, tables and chairs will not be furnished. Vendors must be in place by 9:30AM – Parade starts at 10:00AM. Tear down at 3:00PM. Please supply a basic menu and prices with your donation. A Donation of $100.00 is required for those registrations completed and entered via this web site prior to October 1, 2012 and $150.00 for those entered after October 1, 2012.
I Agree	Release of Claims In consideration of acceptance of this registration to participate in the Daisy Mountain Veterans’ Day Parade and Picnic, I, the undersigned, intending to be legally bound, for myself, heirs, executors and administrators, waive and release any and all rights and claims for the Daisy Mountain Veterans’ Committee, American Legion Post 128 and its members, Veterans of Foreign Wars Post 12031 and its members, Anthem, Maricopa County or any respective agents, volunteers and employees connected with this event.

Your participation will not be effective until your invoice, the last page of the Vendor Packet, and check have been received.
